At Design A Life, our Practitioners undertake a comprehensive, collaborative and person centred approach. We assess, plan and provide intervention that focuses on addressing the person’s needs.
We focus on working with families and carers so together we can develop a shared understanding about the person.
It is so important that we uphold the person’s rights to better assist them to develop personal relationships, improve their health, be more active in the community, or develop personally.

What is Personal Information and why do we collect it?

Design A Life will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information.

It is important to us that your Personal Information is up to date. We will take reasonable steps to make sure that your Personal Information is accurate, complete and up-to-date.
